肺癌患者腔镜术后持续性漏气影响因素的范围综述

摘要:目的 分析肺癌患者腔镜术后持续漏气的影响因素,为避免发生持续性漏气干预方案的设计提供参考。方法 检索中国知网、万方、维普、中国生物医学文献服务系统、PubMed、Web of Science、Embase、The Cochrane Library等数据库中关于肺癌术后持续漏气的相关因素的病例对照研究和队列研究,横断面研究,检索时限为建库至2025年8月。结果 共纳入12篇文献,9017例肺癌腔镜术后患者,确定了20个与肺癌患者腔镜术后持续性漏气相关的影响因素,包括⑴患者自身因素:人口学因素:性别、年龄。既往史:吸烟史。肺质量因素:呼吸合并症、FEV1/FVC、LER、ER、肺裂发育不良。合并基础疾病:心血管疾病。营养状态:BMI、FFMI、血清白蛋白水平、前蛋白水平。⑵病理因素:肿瘤分期、肺癌类型。⑶手术因素:手术类型、手术时间、胸腔黏连松解术、术中估计失血量。⑷医生因素:手术经验。结论 肺癌腔镜术后持续性漏气的发生涵盖多重影响因素,涉及患者自身因素、病理因素、手术因素、医生经验因素等多个维度。医护人员应加强肺癌术后持续漏气高危患者的识别与管理,针对上述多种影响因素提前采取措施,以减少肺癌患者腔镜术后持续漏气的发生,改善患者预后。
